人工流产术对无生育史之女性特别痛苦,为使女性在无痛苦条件下受术,笔者应用异丙酚、芬太尼用于无痛流产60例,经观察效果较为满意,现报道如下。1 资料与方法1.1 ASAⅠ-Ⅱ级无生育史女性60例,早期妊娠45-60天,年龄20-36岁,体重45-70kg,无明显心肺疾病史,手术时间1-6分钟。1.2 方法:①术前常规禁饮食,术前30分钟肌注阿托品0.5mg,术前5分钟由输血器壶中注入芬太尼1μg/kg,麻黄硷0.1mg/kg,5分钟后由输血器壶中注入异丙酚1.8mg/kg,以60D/min的速度滴注,如手术未完,隔3分钟入壶40mg继续滴注,至手术结束。术中常规吸氧,监测血压、脉搏、呼吸。②以睫毛反射消失为神志消失的标志,术后呼之睁眼为意识恢复,能按指令准确指鼻作为定向力恢复,21例受术者在意识消失前和意识恢复前有梦呓。
