目的探讨早孕期胚胎存活而妊娠囊小于孕周者与胚胎停止发育的关系。方法回顾性分析2013年7月-2014年3月在我院门诊进行超声诊断且治疗的早孕期妊娠囊小于孕周的孕妇39例,随访其妊娠结局。结果 39例妊娠囊超声检查最大直径11 mm,最小直径3 mm,其中无卵黄囊者11例,有卵黄囊者28例,卵黄囊大于8 mm者5例,3~8 mm者15例,小于3 mm者8例。超声诊断1~2周后复查超声,发现胚胎停止发育者24例,均为无卵黄囊、卵黄囊大于8 mm或小于3 mm者。15例胚胎存活,均为卵黄囊大小在3~8 mm者,且妊娠到足月。结论早孕妊娠囊小于孕周伴无卵黄囊、卵黄囊大于8 mm或小于3 mm者易发生胚胎停止发育;妊娠囊小于孕周但卵黄囊大小在3~8 mm者预后较好,超声检查能为临床医生及患者提供胚胎预后发育情况,早期甄别胚胎停止发育。
Objective To investigate the relationship between the survival of embryos in early pregnancy and the gestational sac less than gestational weeks to stop the development of embryos. Methods A retrospective analysis of 39 pregnant women with gestational sac in the first trimester who were diagnosed by ultrasound in our hospital from July 2013 to March 2014 was less than gestational age. The pregnancy outcome was followed up. Results The maximum diameter of 39 gestational sacs was 11 mm and the minimum diameter was 3 mm. There were 11 cases without yolk sac, 28 cases with yolk sac, 5 cases with yolk sac more than 8 mm and 15 cases with 3 ~ 8 mm, less than 3 mm in 8 cases. Ultrasound diagnosis 1 to 2 weeks after the review of ultrasound and found that embryos stop development in 24 cases, no yolk sac, yolk sac more than 8 mm or less than 3 mm. Fifteen embryos survived, with yolk sac size of 3 to 8 mm and pregnancy to term. Conclusions The gestational sac in the first trimester is smaller than the gestational age without yolk sac and the yolk sac is larger than 8 mm or smaller than 3 mm. The embryo sac is less than the gestational age but the yolk sac size is 3-8 mm, the prognosis is good. To provide clinicians and patients with embryonic prognosis, early screening of embryos to stop development.
