目的 :探讨青光眼滤过手术中使用透明质酸钠的临床效果。方法 :对 2 12例 (2 3 1眼 )各类青光眼行小梁切除术中前房内注入 1%透明质酸钠并保留 ,观察其并发症及效果 ,并抽出 5 0例随访平均 1 2年 ,此前 5 0例未使用透明质酸钠者为对照组 ,平均随访 1 1年。结果 :治疗组术后第一天前房积血发生率 4 2 % ,对照组 18% ;术后浅前房 5 6% ,对照组 3 6% ;脉络膜脱离 2 6% ,对照组14 % ;眼压 :术后 2 4小时平均 14 5mmHg ,对照组 6 7mmHg ,出院时眼压平均 11 3mmHg ,对照组成 12 5mmHg ;术眼视力 ;术后次日 74 4% ,出院时 87 9%的患者达到或超过术前 ,对照组则分别为 44 %及 68% ;远期效果实验组完全成功率 84% ,对照组 64 %。除出院时眼压两组差异无显著性外 (P <0 0 5 ) ,其它皆有显著性差异 (P <0 0 1)。结论 :青光眼滤过手术中使用透明质酸钠明显减少了并发症 ,提高了手术成功率
Objective: To investigate the clinical effect of sodium hyaluronate used in glaucoma filtration surgery. Methods: 2,12 cases (2131 eyes) of all kinds of glaucoma trabeculectomy were injected with 1% sodium hyaluronate into the anterior chamber and the complications were observed. The average follow-up of 50 Year, after the previous 50 cases of non-use of sodium hyaluronate for the control group, with an average follow-up of 11 years. Results: The incidence of hyphema in the treatment group was 42% on the first postoperative day, 18% in the control group, 56% in the shallow anterior chamber, 36% in the control group, 26% in the choroidal detachment group and 14% in the control group. Intraocular pressure: an average of 14 5mmHg 24 hours after surgery, the control group 6 7mmHg, the average IOP at discharge 11 3mmHg, the control group composed of 12 5mmHg; visual acuity; 74 4% the day after surgery, discharged 87 9% of patients Or more than the preoperative and control groups were 44% and 68% respectively; the long-term effect of the experimental group, the complete success rate of 84%, 64% of the control group. There was no significant difference between the two groups (P <0 05) except for the IOP at discharge. There were significant differences between the two groups (P 0 01). Conclusion: The use of sodium hyaluronate in glaucoma filtration surgery significantly reduced complications and improved the success rate of surgery
