Extramedullary leukemia remains a major complication of acute lymphoblastic leukemia (ALL). Ocular leukemia infiltration is rare and usually occurs concurrently with central nervous system recurrence. The authors report that 2 patients who did not receive treatment for 2 months and 4 months had an anterior segment leukemia infiltration as the only site of recurrence. Patient A: The right eye red pain and photophobia, was diagnosed with uveitis, giving atropine and corticosteroids eye drops improved. The examination showed fixation and irregularities of the right pupil, diffuse hypertrophy of the iris and pale, uveitis, and a small amount of hypopyon. The diagnosis of leukemia infiltration in the right anterior segment was confirmed by anterior chamber puncture and iris biopsy. Right anterior chamber and iris with radiotherapy
