目的系统评价神经元特异性烯醇化酶(NSE)对恶性胸腔积液的诊断价值。方法计算机检索h e Cochrane Library(2012年第1期)、EMbase、MEDLINE、CBM、CNKI、WanFang Data和VIP数据库,查找有关NSE对恶性胸腔积液诊断价值的研究,检索时限均为从建库至2012年1月。由2名评价员按纳入与排除标准独立筛选文献、提取资料和评价质量后,采用Meta-DiSc 1.4软件进行合并分析。结果最终纳入12个研究。Meta分析结果显示:SPE合并=0.79[95%CI(0.76,0.84)]、SEN合并=0.55[95%CI(0.51,0.59)]、+LR=3.2[95%CI(1.94,5.29)]、–LR=0.58[95%CI(0.45,0.74)]、DOR合并=7.56[95%CI(3.74,15.30)]、SROC曲线下面积(AUC)为0.813?1。结论 NSE对诊断恶性胸腔积液具有一定的临床价值,可用于良恶性胸腔积液的鉴别诊断。
Objective To evaluate the value of neuron-specific enolase (NSE) in the diagnosis of malignant pleural effusion. Methods A computer-based search of the He Cochrane Library (2012 issue 1), EMbase, MEDLINE, CBM, CNKI, WanFang Data and VIP databases was conducted to search for the diagnostic value of NSE in malignant pleural effusion. Meta-DiSc 1.4 software was used for the combined analysis. The results eventually included 12 studies. Meta-analysis results showed that SPE incorporation was 0.79 [95% CI (0.76,0.84)], SEN incorporation was 0.55 [95% CI (0.51,0.59)] and LR was 3.2 (95% CI 1.94,5.29) LR = 0.58 [95% CI (0.45,0.74)], DOR combination = 7.56 [95% CI (3.74,15.30)] and the area under the curve of SROC (AUC) was 0.813? Conclusion NSE has certain clinical value in the diagnosis of malignant pleural effusion and can be used in the differential diagnosis of benign and malignant pleural effusions.
