近年来,内镜下食管静脉曲张套扎治疗技术巳引起广泛关注,我们于1995年6月～1997年7月对22例肝硬化门脉高压伴食管静脉曲张患者进行内镜下套扎一硬化联合治疗获较好效果。现报道如下: 1 方法与病例选择 1.1 器械 套扎器由天津市天医医用硅橡胶制品厂生产,使用Olympus GIF-XO_(20)胃镜和NM-3K注射针。 1.2 方法 常规胃镜前准备,将塑料套管套在胃镜上插入胃镜,观察食管静脉曲张的程度和部位,确定套扎的静脉后退出胃镜。安装套扎器,将装有套扎器的胃镜重新插入食管,直视下将曲张静脉吸入内柱圈内,在负压吸引状态下当视野呈一片红色时,拉紧牵引线使硅椽胶圈弹出,套入部分曲张静脉。2～3周后重复治疗。将曲张静脉套扎后,在套扎上方的曲张静脉旁双侧各注射5％鱼肝油酸钠1～2ml,特别注意在较小的曲张静脉旁两侧注射硬化剂。
