患儿,男,2009年10月23日出生。2011年10月24日9:00左右,来我院预防接种门诊接种疫苗。接种前经体检健康状况良好,无接种禁忌证,并请家长在知情同意书签名后,接种流行性乙型脑炎减毒活疫苗(Japanese Encephalitis Atteduated Live Vaccine,JEV-L)(武汉生物制品研究所生产,批号201009065-1,有效期至2012年4月23日),在右上臂外侧三角肌下缘附着处皮下注射,注射剂量0.5ml。预防接种人员资质、器材及接种过程均符合《预防接种工作规范》。接种后留观30min,未发现
