目的调查急性脑卒中住院患者中糖尿病和糖调节受损的情况。方法选取2008年8月~2009年12月符合急性脑卒中诊断纳入标准的住院患者为研究对象,共收集有效病历85份,入院24~48 h检测空腹血糖、糖化血红蛋白等指标,入院后24 h内与发病10~14 d分别进行mRs、NIHSS、GCS评分。入院前未诊断糖尿病的患者于发病7~14 d病情稳定后行标准75g葡萄糖耐量试验(OGTT),分别测定0、2 hPG值;已确诊糖尿病的患者仅做餐后血糖监测,不做OGTT试验。检测后分为糖尿病组34例、糖调节受损组23例及正常糖耐量组28例。糖代谢分类参考2006年WHO糖尿病和中间型高血糖诊断标准。结果急性脑卒中住院患者中16例住院前已明确诊断为糖尿病,占15.3%。69例未诊断为糖尿病的患者经OGTT检查,其中新诊断糖尿病18例,占26.1%,糖调节受损23例,占33.3%;糖耐量正常28例,占40.6%。结论急性脑卒中住院患者中伴随着较高比例的糖代谢异常,通过常规检测空腹血糖、糖化血红蛋白和OGTT试验可以早期准确地发现这些患者。
Objective To investigate the prevalence of diabetes and impaired glucose regulation in hospitalized patients with acute stroke. Methods From August 2008 to December 2009, hospitalized patients who met the criteria of acute stroke diagnosis were enrolled. A total of 85 valid medical records were collected. Fasting blood glucose and glycosylated hemoglobin were measured 24 to 48 hours after admission. Internal and onset of 10 ~ 14 d were mRs, NIHSS, GCS score. The patients with undiagnosed diabetes before admission were given standard 75g glucose tolerance test (OGTT) after 7-14 days of disease onset, and the patients were diagnosed with diabetes mellitus only for postprandial blood glucose monitoring and no OGTT test . After the test, 34 cases were divided into diabetic group, 23 cases with impaired glucose regulation and 28 cases with normal glucose tolerance group. Refer to 2006 WHO classification of diabetes mellitus and intermediate hyperglycemia diagnostic criteria. Results Sixteen inpatients with acute stroke had a definite diagnosis of diabetes before hospitalization, accounting for 15.3%. Sixty-nine patients with undiagnosed diabetes were examined by OGTT. Among them, 18 cases were newly diagnosed with diabetes mellitus (26.1%), 23 cases were impaired glucose regulation (33.3%), and 28 cases were normal glucose tolerance (40.6%). Conclusion Inpatients with acute stroke are accompanied by a high proportion of abnormal glucose metabolism. These patients can be accurately and early discovered by routine tests of fasting blood glucose, glycosylated hemoglobin and OGTT.
