目的观察单肺通气对围手术期患者TNF-α、IL-6、IL-8和SOD的影响。方法 2008年1月至2009年1月,46例ASAⅠ~Ⅱ肺癌患者,随机分为两组,A组23例,均采用单肺通气,B组23例,进行间断双肺通气,观察期间两组血清TNF-α、IL-6、IL-8和SOD的水平变化。结果两组患者T3、T4时TNF-α、IL-6和IL-8水平明显高于T1时水平,而SOD水平明显下降,差异有统计学意义(P<0.05)。A组患者T4时TNF-α、IL-6和IL-8水平明显高于同期B组水平,SOD水平明显低于同期B组水平,两组比较差异有统计学意义(P<0.05)。结论单肺通气期间炎性TNF-α、IL-6、IL-8和SOD水平明显发生变化,而较长时间间断双肺通气可减轻炎性反应。
Objective To observe the effect of single lung ventilation on the levels of TNF-α, IL-6, IL-8 and SOD in patients undergoing perioperative period. Methods From January 2008 to January 2009, 46 patients with ASAⅠ ~ Ⅱ lung cancer were randomly divided into two groups. Group A (23 cases) received single lung ventilation and group B (23 cases) received intermittent double lung ventilation. During the observation period, Serum levels of TNF-α, IL-6, IL-8 and SOD were measured. Results The levels of TNF-α, IL-6 and IL-8 at T3 and T4 in both groups were significantly higher than those at T1 and SOD levels were significantly decreased (P <0.05). The level of TNF-α, IL-6 and IL-8 in group A was significantly higher than that in group B at the same period, while the level of SOD was lower in group A than in group B at the same period. There was significant difference between the two groups (P <0.05). Conclusions The level of inflammatory cytokines TNF-α, IL-6, IL-8 and SOD during single lung ventilation change obviously, and prolonged interval pneumoconiosis can reduce the inflammatory reaction.
