目的:探讨沙盘游戏对癫痫患儿心理问题的治疗作用。方法:采用随机对照研究方法,随机纳入在重庆医科大学附属儿童医院神经内科门诊确诊为癫痫的40例患儿,排除无严重躯体疾病及精神病史。40例癫痫患儿随机分为实验组(20例)与对照组(20例)。实验组在药物治疗的同时进行20周沙盘游戏心理治疗,对照组仅进行药物治疗。采用SCL-90症状自评量表为测评工具,对治疗前后的癫痫组患儿进行评估。结果:(1)癫痫组躯体化[(1.97±0.59)分]、强迫[(2.85±0.55)分]、人际关系[(3.12±0.74)分]、抑郁[(3.22±0.85)分]、焦虑[(3.15±0.66)分]、敌对[(3.93±0.94)分]、恐怖[(2.34±0.72)分]、偏执[(3.15±0.81)分]、精神病性[(2.93±0.78)分]指数得分明显高于国内常模,差异具有统计学意义(P<0.05)。(2)治疗后实验组躯体化、强迫、人际关系、抑郁、焦虑、敌对、恐怖、偏执、精神病性指数得分均较对照组有明显降低(P<0.05)。(3)治疗后实验组躯体化、强迫、人际关系、抑郁、焦虑、敌对、恐怖、偏执、精神病性指数得分均较治疗前有明显降低(P<0.05)。结论:癫痫患儿有心理障碍。沙盘游戏心理治疗能够有效缓解癫痫患儿的心理行为问题。
Objective: To explore the therapeutic effect of sand table game on psychological problems in children with epilepsy. Methods: A randomized controlled study was randomly included in the 40 cases of epilepsy diagnosed as pediatric epilepsy in the Department of Neurology, Children’s Hospital Affiliated to Chongqing Medical University, excluding the history of no serious physical diseases and mental illness. Forty children with epilepsy were randomly divided into experimental group (20 cases) and control group (20 cases). The experimental group in the drug treatment at the same time 20 weeks of psychological treatment of sand table games, the control group only for drug treatment. SCL-90 Symptom Checklist for the evaluation tool for the evaluation of children with epilepsy before and after treatment. (1.97 ± 0.59), forced (2.85 ± 0.55), interpersonal (3.12 ± 0.74), depression (3.22 ± 0.85), anxiety [(3.15 ± 0.66) points], (3.93 ± 0.94) points of hostility, (2.34 ± 0.72) points of paranoidness, (3.15 ± 0.81) points of paranoia and (2.93 ± 0.78) points of psychosis Score was significantly higher than the domestic norm, the difference was statistically significant (P <0.05). (2) The scores of somatization, compulsion, interpersonal relationship, depression, anxiety, hostility, terror, paranoid and psychotic index in the experimental group were significantly lower than those in the control group after treatment (P <0.05). (3) The scores of somatization, compulsion, interpersonal relationship, depression, anxiety, hostility, terror, paranoid and psychotic index in the experimental group after treatment were significantly lower than those before treatment (P <0.05). Conclusion: Children with epilepsy have mental disorders. Sandplay game psychotherapy can effectively alleviate the psychological behavior of children with epilepsy.
