[目的]观察消化道肿瘤并发抑郁症患者血清IL-6、IL-8水平,分析影响因素及中药白龙解郁颗粒的临床疗效。[方法]采用SDS自评量表及HAMD他评量表对肿瘤患者进行评分,分析肿瘤并发抑郁症的相关因素;采用Elisa方法比较肿瘤并发抑郁组与肿瘤非抑郁组患者血清IL-6及IL-8水平;并给予肿瘤并发抑郁症患者白龙解郁颗粒治疗4周,观察其对肿瘤并发抑郁症患者SDS、HAMD评分及血清IL-8水平的影响。[结果]消化道肿瘤并发抑郁症患者的血清IL-8水平为(245.15±97.60)pg/ml,高于肿瘤非抑郁症组(144.38±113.95)pg/ml及正常对照组(13.01±3.49)pg/ml(P<0.05),各组血清IL-6的水平差异无统计学意义(P>0.05)。消化道肿瘤并发抑郁症的SDS评分与血清IL-8的水平及肿瘤分期呈正相关。白龙解郁颗粒治疗4周后消化道肿瘤并发抑郁症患者的SDS、HAMD评分及血清IL-8水平分别为(44.79±8.73)分、(11.05±3.92)分及(172.72±72.61)pg/ml,均较治疗前下降(P<0.05)。[结论]消化道肿瘤并发抑郁症患者存在细胞因子IL-8异常激活;血清IL-8水平及肿瘤分期是并发抑郁症的影响因素,中药白龙解郁颗粒具有抗抑郁作用,调节血清IL-8是其可能的作用机制。
[Objective] To observe the serum levels of IL-6 and IL-8 in patients with digestive tract tumors complicated with depression, analyze the influencing factors and the clinical efficacy of Bailong Jieyu granule. [Methods] The patients with cancer were scored by SDS self-rating scale and HAMD-scale, and the related factors of depression complicated by tumor were analyzed. The Elisa method was used to compare the serum levels of IL-6 and IL -8 levels. The patients with tumor complicated with depression were treated with Bailong Jieyu granule for 4 weeks, and their effects on SDS, HAMD score and serum IL-8 level in patients with tumor complicated with depression were observed. [Results] The level of serum IL-8 in patients with digestive tract tumor complicated with depression was (245.15 ± 97.60) pg / ml, higher than that in non-depressed patients (144.38 ± 113.95 pg / ml) and normal control group (13.01 ± 3.49) pg / ml (P <0.05). There was no significant difference in the level of serum IL-6 in each group (P> 0.05). SDS score of gastrointestinal cancer complicated with depression was positively correlated with serum IL-8 level and tumor stage. The SDS, HAMD score and serum IL-8 levels in patients with gastrointestinal cancer complicated with depression after treatment with Bailu Jieyu granule for 4 weeks were (44.79 ± 8.73), (11.05 ± 3.92) and (172.72 ± 72.61) pg / ml, all lower than before treatment (P <0.05). [Conclusion] Abnormal activation of cytokine IL-8 exists in patients with digestive tract tumor complicated with depression. Serum IL-8 level and tumor stage are the influencing factors of concurrent depression. Chinese herb Bailong Jieyu granule has antidepressant effect and regulates serum IL- 8 is its possible mechanism of action.
