目的:探讨游离旋髂浅动脉分叶穿支皮瓣在临床创面修复中的应用方法及效果。方法:自2018年11月至2019年12月,我们共收治6例手及足踝部软组织缺损患者,手部创面4例,踝关节周围创面1例,足背创面1例。1例为单个创面,创面面积为10 cm×13 cm;4例为相邻两个创面;1例为相邻三个创面,创面面积为5 cm×2 cm~7 cm×6 cm。应用游离旋髂浅动脉分叶穿支皮瓣修复创面,5例为双叶皮瓣,1例为三叶皮瓣,皮瓣每一叶面积为5.5 cm×2.2 cm~10.5 cm×7.0 cm。腹股沟供区均直接缝合。术后观察临床疗效。结果:术后6例皮瓣全部存活,其中1例双叶皮瓣的一叶早期出现血供不足,未做处理,一周后皮瓣血供改善,伤口均Ⅰ期愈合。术后随访2~14个月,平均7个月,所有皮瓣外形良好,不臃肿。结论:采用游离旋髂浅动脉分叶穿支皮瓣可同时修复多个创面缺损,皮瓣供区隐蔽,供区可直接缝合,损伤小,是一种较理想的修复方法。“,”Objective:To explore the application method and clinical efficacy of free superficial circumflex iliac artery perforator lobulated flap in wound repair.Methods:From November 2018 to December 2019, 6 cases of hand and foot ankle soft tissue defects were treated, including 4 cases of hand wounds, 1 case of ankle joint wounds and 1 case of foot dorsal wounds. One case was a single wound with an area of 10 cm×13 cm, 4 cases with two adjacent wounds, and one case with three adjacent wounds with an area of 5 cm×2 cm to 7 cm×6 cm. The free superficial circumflex iliac artery perforator lobulated flap was used to repair the wounds. 5 cases were bilobated flaps and 1 case was trilobated flaps. The area of each flap was 5.5 cm×2.2 cm to 10.5 cm×7.0 cm. All the donor sites were sutured directly. The clinical efficacy was observed after operation.Results:All the flaps survived after operation. One of the bilobated flaps had insufficient blood supply at the early stage and was not treated. One week later, the blood supply of the flap was improved and the wound healed. The follow-up time ranged from 2 to 14 months with an average of 7 months. All the flaps had good appearance and were not bulky.Conclusion:The free superficial circumflex iliac artery perforator lobulated flap can repair multiple wound defects at the same time. The donor site of the flap is concealed, and the donor site can be sutured directly with less damage. It is an ideal repair method.
