乙型肝炎是全人类所面对的一种可以引起严重后遗症的健康问题。尽管已经开发了一些治疗药物,但对其的治疗仍在继续变得复杂化。目前,对乙型肝炎的治疗措施已经从以前可采用的有限的疗法改进为在几个族群中采取终身病毒抑制。之所以改变对这一人类健康问题的应对策略,不仅是因为研发出来了一些耐受性较好的口服药物,而且还因为人们越来越清楚地认识到病毒复制活跃会引发严重后遗症,如肝硬化、肝功能衰竭及肝细胞癌。然而,治疗也可能带来一定的风险,其中包括不良反应、花费及耐药性的产生。另外,围绕几个临床问题的争论仍在继续,即哪些患者符合接受治疗的条件,哪种疗法最为适合以及何时可以停止治疗。该篇综述文章(Nidhi A.Management of hepatitis B virus[J].J Antimicrob Chemother,2008,62(2):224-228)将针对上述问题和用于支持现行治疗指南的参考文献的局限性进行讨论。
Hepatitis B is a health problem that humans can face with serious sequelae. Although some therapeutic drugs have been developed, their treatment continues to be complicated. Currently, treatment of hepatitis B has been improved from the limited treatment available previously to take lifelong virus suppression in several ethnic groups. The reason why this response to this human health problem has changed is not only because some well-tolerated oral medications have been developed, but also because people are increasingly aware that an active viral replication can lead to serious sequelae such as liver Sclerosis, liver failure and hepatocellular carcinoma. However, treatment may also bring some risks, including adverse reactions, cost and drug resistance. In addition, the debate surrounding several clinical issues continues, namely, which patients are eligible for treatment, which treatment is best suited, and when treatment can be stopped. J Antimicrob Chemother, 2008, 62 (2): 224-228) addresses the above issues and the limitations of references used to support current therapeutic guidelines discuss.
