目的探讨联合检测血清同型半胱氨酸(Hcy)和脑钠肽(BNP)在糖尿病肾病(DN)早期诊断中的应用价值。方法糖尿病患者86例,按尿蛋白排泄率(UAER)分为正常蛋白尿(A组,32例)、微量蛋白尿(B组,29例)和临床蛋白尿(C组,25例)三组;选取同期健康体检者40例为对照(D组),分别检测和比较四组患者血清Hcy、BNP和血尿素氮(BUN)水平。结果 A、B和C三组血清BNP和Hcy水平均高于D组(P<0.05);随UAER增加,糖尿病患者BNP和Hcy水平升高;联合检测Hcy和BNP的阳性率高于各单项指标检测(P<0.05);血清Hcy和BNP水平与UAER呈正相关(r=0.804和0.761,P<0.05)。结论血清Hcy和BNP水平与UAER密切相关;联合检测Hcy和BNP较单一指标更能够判断糖尿病患者肾脏损害程度,对DN的早期诊断有较好的临床应用价值。
Objective To investigate the value of combined detection of serum homocysteine (Hcy) and brain natriuretic peptide (BNP) in the early diagnosis of diabetic nephropathy (DN). Methods Eighty-six patients with diabetes were divided into three groups according to urinary protein excretion rate (UAER): normal proteinuria (group A, 32 cases), microalbuminuria (group B, 29 cases) and clinical proteinuria (group C, 25 cases) 40 healthy subjects were selected as the control group (D group). The levels of serum Hcy, BNP and BUN in the four groups were detected and compared. Results Serum levels of BNP and Hcy in groups A, B and C were significantly higher than those in group D (P <0.05). With the increase of UAER, the levels of BNP and Hcy in diabetic patients were increased. The positive rates of Hcy and BNP were higher than those of individual indexes (P <0.05). Serum Hcy and BNP levels were positively correlated with UAER (r = 0.804 and 0.761, P <0.05). Conclusions Serum Hcy and BNP levels are closely related to UAER. The combined detection of Hcy and BNP can better determine the degree of renal damage in diabetic patients than single indicators and has a good clinical value in the early diagnosis of DN.
