社区卫生服务的筹资与补偿机制是社区卫生服务可持续发展的瓶颈问题 ,亟待建立与完善。作者在分析北京市社区卫生服务筹资管理存在的问题的基础上 ,提出了实行有偿服务和政府购买服务相结合的筹资与补偿机制和 8个筹资渠道。对于社区卫生服务 ,建议政府要强化职能 ,制定合理的补偿政策 ,财政投入应侧重预防保健、健康教育和人才培养等方面 ,将社区卫生服务纳入医疗保险制度管理 ,制定并完善社区卫生服务价格 ,建立多种层次、多种形式的社区卫生服务体系及结构 ,努力拓展社区卫生服务机构生存发展空间。
The fundraising and compensation mechanism of community health services is a bottleneck problem for the sustainable development of community health services, and it needs to be established and improved. Based on the analysis of the problems existing in the financing management of community health services in Beijing, the author proposes a fundraising and compensation mechanism and eight financing channels that combine paid services with government purchase services. For community health services, it is recommended that the government should strengthen its functions and formulate reasonable compensation policies. Financial investment should focus on preventive health care, health education, and personnel training. Community health services should be included in the medical insurance system management, and community health service prices should be established and improved. Establish a variety of levels and forms of community health service system and structure, and strive to expand the survival and development space of community health service institutions.
