目的:探讨窒息新生儿甲状腺功能的变化及意义。方法:对63例窒息新生儿(窒息组)和30例同期出生的健康新生儿(对照组)于生后24~72 h取股静脉血,用竞争性放射免疫法测定三碘甲腺原氨酸(T3)、甲状腺素(T4)、促甲状腺激素(TSH)值,并进行对比观察。结果:窒息新生儿T3、T4值显著低于对照组(t=2.210、11.896,P=0.031、0.000),TSH高于对照组(t=4.961,P=0.000);重度组T4显著低于轻度窒息组(t=2.048,P=0.045),TSH高于轻度窒息组(t=2.628,P=0.011),T3两组无明显差异(t=0.730,P=0.468)。结论:新生儿窒息后可出现甲状腺功能低下,以T4降低、TSH升高为先,甲状腺激素降低的程度与病情严重程度成正比。T4和TSH是敏感指标,T3降低对预警病情严重有意义。
Objective: To investigate the changes and significance of thyroid function in neonates with asphyxia. Methods: 63 cases of asphyxia neonatorum (asphyxia group) and 30 healthy newborn infants (control group) were enrolled in this study. Blood samples of femoral vein were collected from 24 to 72 hours after birth, and radioimmunoassay was used to determine the contents of triiodothyronine Acid (T3), thyroxine (T4), thyroid stimulating hormone (TSH) values, and compared. Results: The values of T3 and T4 in neonates with asphyxia were significantly lower than those in control group (t = 2.210,11.896, P = 0.031,0.000), TSH was higher than that in control group (t = 4.961, P = 0.000) (T = 2.048, P = 0.045), TSH higher than mild asphyxia group (t = 2.628, P = 0.011), there was no significant difference between the two groups (t = 0.730, P = 0.468). Conclusion: Neonatal asphyxia can occur after hypothyroidism with T4 decreased, TSH increased first, the degree of thyroid hormone reduction is proportional to the severity of the disease. T4 and TSH are sensitive indicators, T3 reduced the severity of the warning meaningful.
