目的探讨多种细胞因子在系统性红斑狼疮(Systemic lupus erythematosus,SLE)合并妊娠中的相互影响,为细胞因子作为SLE合并妊娠的病情活动程度及预后判断的监测指标提供依据。方法取SLE妊娠组30例及健康妊娠组30例,妊娠早期、中期、晚期、分娩后的血清置-80℃冻存;采用多重荧光微球技术,检测IL-2、IFN-γ、IL-6、IL-8、IL-10、IL-12、IP-10、TNF-α等8种SLE相关细胞因子。结果细胞因子IFN-γ、IL-6、IL-8、IL-10、IL-12、IP-10、TNF-α在SLE妊娠妇女孕早期、中期、晚期及产后的血清水平较健康对照组各产程高,但仅3种细胞因子IL-8、IL-10、IP-10差异存在统计学意义(P<0.05)。同时,这3种细胞因子在活动期SLE妊娠组孕早期、中期、晚期及产后的血清水平,均较稳定期SLE妊娠组各产程高,差异存在统计学意义(P<0.05)。结论细胞因子的表达在SLE妇女对妊娠的反应变化情况与正常妇女相似,其中SLE相关因子IL-8、IL-10、IP-10在各产程较正常妊娠妇女偏高,特别是在活动期SLE患者妊娠时,IL-8、IL-10、IP-10水平与SLE患者的狼疮病情活动度成正相关,可以作为SLE活动的指标,为临床监测作指导。
Objective To investigate the interaction between various cytokines and systemic pregnancy in patients with systemic lupus erythematosus (SLE), and to provide basis for cytokines as monitoring indicators of disease activity and prognosis in SLE complicated with pregnancy. Methods The serum levels of IL-2, IFN-γ and IL-10 in sera of pregnant women and 30 healthy pregnant women were measured at the early, middle, late pregnancy and after delivery. 6, IL-8, IL-10, IL-12, IP-10, TNF-α and other 8 kinds of SLE related cytokines. Results Serum levels of IFN-γ, IL-6, IL-8, IL-10, IL-12, IP-10 and TNF-α in pregnant women with SLE in early pregnancy, There were significant differences in IL-8, IL-10 and IP-10 among the three cytokines (P <0.05). At the same time, the serum levels of these three kinds of cytokines in the active SLE pregnancy group were higher than those in the stable SLE pregnancy group (P <0.05). Conclusion The expression of cytokines in SLE women respond to changes in pregnancy similar to normal women, including SLE related factors IL-8, IL-10, IP-10 in the labor process than normal pregnant women, especially in active SLE The level of IL-8, IL-10 and IP-10 in patients with pregnancy is positively correlated with the activity of lupus in patients with SLE, and can be used as an index of SLE activity to guide the clinical monitoring.
