本文对45例血栓闭塞性脉管炎(TAO)截肢标本中血管分节段进行了病理组织学观察,见病变血管示慢性非特异性炎症改变,在活动期,以渗出为主,见大量淋巴细胞浸润,并伴有血管内膜受损;以后出现继发性病变,包括血栓形成、机化、管腔闭塞。对部分新鲜标本的血管进行了免疫病理学研究,包括免疫荧光和免疫酶标法,在活动性炎的血管壁上见有免疫球蛋白(IgM、IgG、IgA)和补体(C_(1q)、C_3)存在,说明局部有免疫复合物沉积。以上结果提示:TAO的发生与免疫有关,是一种自身免疫性疾病。
