患者女性,42岁.诉上腹部疼痛五天,放射至下腹部,伴有恶心,呕吐,便秘入院.疼痛病初剧烈,24小时后稍有缓解,随即遍及整个腹部,伴有寒战、发热.检查发现腹部紧张,压痛和反跳痛,右下腹部有一压痛肿块;直肠检查发现右下腹有一明显触痛的腹膜包块.入院时体温39.4℃,寒战,皮肤潮红发热.血压120/80毫米汞柱.开始了一个疗程的抗菌素治疗后,血压上升至160/70毫米汞柱,脉率和尿量减少.随后皮肤潮冷,血压下降到60/0毫米汞柱,脉搏150次/分.
Patient female, age 42, complained of abdominal pain for five days, radiating to the lower abdomen, accompanied by nausea, vomiting, constipation admitted to the hospital.Acute pain initially severe, 24 hours after a slight relief, then throughout the abdomen, accompanied by chills, fever. Examination revealed abdominal tension, tenderness and rebound tenderness, the right lower abdomen has a tender mass; rectal examination revealed a right tenderness of the right lower quadrant peritoneal mass .Physiological temperature 39.4 ℃, chills, skin flushing fever. Blood pressure 120/80 mmHg After a course of antibiotic treatment, blood pressure rose to 160/70 mmHg and pulse rate and urine output decreased, followed by a tidal cold with blood pressure dropping to 60/0 mmHg and a pulse rate of 150 beats / min.
