目的探讨经闭孔无张力尿道中段悬吊术(TVT-O)治疗女性压力性尿失禁的临床疗效。方法选取解放军第59中心医院2013年3月-2014年3月收治的50例压力性尿失禁女性患者行TVT-O治疗。观察患者的手术时间、出血量及治疗效果。患者手术3~12个月后进行随访,观察术后并发症发生情况。结果 50例患者均成功完成手术,均治愈,治疗效果好。平均手术时间为(22.51±5.50)min,出血量为(20.43±9.57)ml,ICI-O-SF评分为(0.57±0.11)分,与术前相比明显下降,差异有统计学意义(t=35.564,P<0.01)。术后出现排尿困难和尿潴留各1例,经处理后症状消失;出现大腿根部慢性疼痛3例,无其他不良反应。结论 TVT-O治疗女性压力性尿失禁疗效确切,操作简便、手术时间短、安全性较高,具有较高临床价值。
Objective To investigate the clinical efficacy of transtibial tension-free mid-urethral suspension (TVT-O) in the treatment of female stress urinary incontinence. Methods Fifty patients with stress incontinence who were treated in the 59th Central Hospital of PLA from March 2013 to March 2014 were treated with TVT-O. Observed the patient’s operation time, bleeding and treatment. Patients were followed up for 3 to 12 months after surgery to observe the postoperative complications. Results All the 50 patients were successfully completed the operation, were cured, the treatment effect is good. The average operation time was (22.51 ± 5.50) min, the bleeding volume was (20.43 ± 9.57) ml and the ICI-O-SF score was (0.57 ± 0.11) points, which was significantly lower than that before operation = 35.564, P <0.01). Postoperative dysuria and urinary retention in 1 case, after treatment, the symptoms disappeared; there were 3 cases of chronic thigh root pain, no other adverse reactions. Conclusion TVT-O treatment of female stress urinary incontinence is effective, easy to operate, short operative time, high safety and high clinical value.
