目的:分析门诊药房调剂差错的原因,并探讨其干预方法与效果。方法:调取门诊药房从2012年1月到2013年12月两年间调剂差错事件作为研究对象,回顾分析调剂差错的发生原因,并通过对比2012年1月到12月(门诊药房调剂干预措施实施前)与2013年1月到12月(门诊药房调剂干预措施实施后)两年间的调剂差错发生情况,探讨干预措施的实施效果。结果:2012-2013两年间,门诊药房药品调剂错误事件共35起,在成调配错误的主要原因包括处方书写不规范,药品名称或包装相似,药品剂量、规格或剂型混淆,药品摆放不规范,药师对处方审核不严格,药师专业知识欠缺等,其中以前三种构成比最高,分别占34.27%、20.00%和17.14%。2013年药品调剂错误事件发生次数显著降低,与2012年相比改善明显(P<0.05)。结论:针对造成门诊药品调配差错事件的发生原因进行针对性干预,有利于提高药品的调配质量,保证患者的用药安全和用药效果,值得临床推广应用。
OBJECTIVE: To analyze the causes of errors in the pharmacy dispensing and to explore the intervention methods and effects. Methods: The outpatient pharmacy was transferred from January 2012 to December 2013 for two years as the research object, and the causes of the adjustment errors were retrospectively analyzed. By comparing the implementation of the outpatient pharmacy adjustment interventions from January to December in 2012, Before January 2013 and December 2013 (after the outpatient pharmacy mediation interventions were implemented) for two years, and to investigate the implementation effect of the interventions. Results: During the two years between 2012 and 2013, outpatient pharmacy dispensing errors occurred for a total of 35 cases. The main reasons for the mismatch between prescription and pharmacy in 2012-2013 included irregular writing of prescriptions, similarity of drug names or packaging, confusion of dose, specification or formulation of medicines, non-standardization of medicines , Pharmacists did not strictly review the prescriptions and lack of professional knowledge of pharmacists. Among them, the proportions of the three former compositions were the highest, accounting for 34.27%, 20.00% and 17.14% respectively. In 2013, the frequency of errors in pharmaceutical preparations was significantly reduced, which was significantly improved compared with 2012 (P <0.05). Conclusion: Targeted interventions aimed at the causes of errors in outpatient medication deployment help to improve the deployment quality of drugs, and ensure the medication safety and medication effect of patients, which is worthy of clinical promotion and application.
