目的探讨单纯性肥胖儿童胰岛素抵抗相关的血浆、游离脂肪酸(FFA)两项指标与儿童胰岛素抵抗的关系。方法选择唐山市妇幼保健院治疗的并被确诊为单纯性肥胖儿童患者50例作为实验对象(实验组),再随机选择正常儿童30例作为参照(对照组),测定所有参加实验的儿童的血浆胰岛素、游离脂肪酸(三酰甘油、总胆固醇等)及卡尼汀(CT)。计算胰岛素抵抗指数(InRI)和胰岛素敏感指数(InSI),测量各个儿童的腰围、臀围及身高体重。计算腰臀比(WHR)及体质量指数(BMI)。结果实验组血浆CT、FFA水平、WHR、TG、胰岛素水平、InRI等方面差异均有统计学意义(P<0.05)。结论实验组儿童与健康对照组儿童相比其身体内血浆CT及FFA和胰岛素抵抗的相关性大,体内的脂肪代谢紊乱,可能与浆CT及FFA的变化有关。
Objective To investigate the relationship between plasma insulin and free fatty acid (FFA) and insulin resistance in children with simple obesity. Methods 50 cases of children with simple obesity who were treated in Tangshan MCH hospital were selected as the experimental group (experimental group), and 30 cases of normal children were randomly selected as the reference group (control group), the plasma of all the participating children Insulin, free fatty acids (triglycerides, total cholesterol, etc.) and carnitine (CT). Insulin resistance index (InRI) and insulin sensitivity index (InSI) were calculated, waist circumference, hip circumference and height and weight of each child were measured. WHR and BMI were calculated. Results There were significant differences in the levels of CT, FFA, WHR, TG, insulin and InRI in the experimental group (P <0.05). Conclusion Children in the experimental group and children in the healthy control group have higher correlations with plasma CT, FFA and insulin resistance than those in the healthy control group. The disorder of fat metabolism may be related to changes of CT and FFA.
