目的探讨星形细胞瘤瘤周水肿、血管内皮生长因子(VEGF)表达程度、微血管密度(microvesseldensity,MVD)和病理分级之间的相关性。方法对33例经手术和病理证实的星形细胞瘤术前分别行常规横断T1WI、T2WI平扫和Gd-DTPA增强后三维T1WI,术后取肿瘤组织行VEGF表达程度和MVD的免疫组化测定,计算每一例肿瘤标本的VEGF表达程度和MVD,将VEGF表达程度分别与瘤周水肿、MVD及病理分级进行相关分析。结果星形细胞瘤病理分级越高,VEGF表达越强(P<0.01);肿瘤周围水肿程度、MVD与VEGF表达强度之间存在密切的关系。结论星形细胞瘤瘤周水肿与VEGF表达强度、MVD具有密切的相关性,因此,在一定程度上可根据MRI所显示的瘤周水肿程度对星形细胞瘤恶性程度和预后作出判断。
Objective To investigate the correlation between the edema of astrocytoma, the expression of vascular endothelial growth factor (VEGF), microvessel density (MVD) and pathological grade. Methods Thirty-three cases of astrocytoma confirmed by operation and pathology were treated with routine T1WI, T2WI and Gd-DTPA enhanced three-dimensional T1WI respectively. The expression of VEGF and MVD in the tumor tissue were detected by immunohistochemistry The expression of VEGF and MVD in each tumor sample were calculated. The correlation between the expression of VEGF and peritumoral edema, MVD and pathological grade were analyzed. Results The higher grade of astrocytoma, the higher the expression of VEGF (P <0.01). There was a close relationship between the degree of edema around the tumor and the expression of MVD and VEGF. Conclusion Astrocytoma with edema and VEGF expression intensity, MVD has a close correlation, therefore, to a certain extent, according to MRI extent of edema showed on the degree of malignancy and prognosis of astrocytoma to make judgments.
