肝细胞和胆管上皮细胞角蛋白(CK)组型不同,前者仅表达CK18和CK8,而后者还表达CK19和CK7.应用识别CK18,CK19的抗体以及层粘连蛋白(LN)抗体,我们把上述二种上皮细胞在形态及CK组型上的差别追溯到了胚胎肝内胆管发生的早期.门静脉分支周围正在向小管状结构转变的不成熟肝细胞开始表达CK19,表明CK19表达是胎肝细胞向胆管上皮分化的标志,是这一过程中的一个重要事件.其意义可能在于适应胆管上皮细胞内复杂的中间丝-桥粒系统.我们还发现,CK19阳性细胞与LN阳性的基底膜样结构之间有极为密切的关系,更重要的是,LN在肝内的出现先于CK19阳性肝细胞的出现.这种位相和时相上的关联强烈提示,门静脉分支周围的LN沉积对肝内胆管的形成与分化起重要诱导作用.
Hepatocytes and cholangiocarcinoma have different keratin (CK) histotypes. The former only express CK18 and CK8, while the latter also express CK19 and CK7. Using the antibodies recognizing CK18 and CK19 as well as the laminin (LN) antibodies, we trace the differences in morphology and CK histology of the two epithelial cells back to the early stages of embryo intrahepatic bile duct formation. It is an important event in this process that CK19 is expressed in the immature liver cells that are transiting to the small tubular structure around the branches of the portal vein, indicating that the expression of CK19 is a marker of differentiation of fetal hepatocytes into the bile duct epithelium. Its significance may lie in adapting to the complex intermediate filament-desmosome system in biliary epithelial cells. We also found that there was a close relationship between CK19-positive cells and LN-positive basal-membrane-like structures. More importantly, the appearance of LN in the liver preceded the appearance of CK19-positive hepatocytes. This association of phases and phases strongly suggests that LN deposition around the branches of the portal vein plays an important inducing role in the formation and differentiation of intrahepatic bile ducts.
