目的评价血浆N末端脑钠素原(NT-pro-BNP)对儿童急性肺损伤(ALI)及急性呼吸窘迫综合征(ARDS)的诊断及预后价值。方法选取30例ALI与ARDS患儿将其作为观察组,同时选取同期20例心源性疾病伴呼吸困难患儿作为对照组,术前分别检测两组患儿肺损伤程度与疾病危重程度,同时检测两组患儿NT-pro-BNP水平,评估其对ALI与ARDS患儿的早期诊断价值及其对患儿预后水平的影响。结果观察组患儿血清NT-pro-BNP水平为(7874.5±3125.6)ng/L,明显高于对照组的(4091.6±1763.3)ng/L,组间对比差异有统计学意义(P<0.05);ALI与ARDS患儿危重程度评分、急性肺损伤程度与血清NT-pro-BNP水平呈正相关(r=0.425、0.368,P<0.05),与患儿1个月生存情况呈负相关(r=-0.301,P<0.05)。结论在ALI与ARDS患儿的早期诊断与预后评估中采用NT-pro-BNP检测,可反馈患儿病情的严重程度,提示患儿的预后水平,为其早期确诊与治疗提供参照,有较高的应用价值。
Objective To evaluate the diagnostic value and prognostic value of plasma N-terminal pro-brain natriuretic peptide (NT-pro-BNP) in children with acute lung injury (ALI) and acute respiratory distress syndrome (ARDS). Methods Thirty children with ALI and ARDS were selected as the observation group. At the same time, 20 cases of cardiogenic disease with dyspnea in the same period were selected as the control group. The degree of lung injury and the severity of the disease in both groups were detected before operation. The levels of NT-pro-BNP in both groups were detected to evaluate their early diagnostic value and prognosis in children with ALI and ARDS. Results The level of NT-pro-BNP in the observation group was significantly higher than that in the control group (7874.5 ± 3125.6 ng / L, 4091.6 ± 1763.3 ng / L, P <0.05) (P <0.05). There was a positive correlation between the severity of ALI and ARDS and the level of serum NT-pro-BNP in children with ALI and ARDS (r = 0.425,0.368, P < -0.301, P <0.05). Conclusion The detection of NT-pro-BNP in the early diagnosis and prognosis of children with ALI and ARDS can feedback the severity of the disease in children and suggest the prognosis of children with early diagnosis and treatment, The application value.
