目的:研究缺血性脑卒中患者血浆中凝血因子Ⅶ(FⅦ)与纤维蛋白原的表达,并阐述其临床意义。方法:FⅦ测定:FⅦ抗原(FⅦ:Ag)采用ELISA法,FⅦ活性(FⅦ:C)采用一阶段凝固法,活化型FⅦ(FⅦa)采用重组可溶性组织因子(r-STF法)。纤维蛋白原(Fbg)测定:采用蕲蛇酶水解-微机自动检测法。结果:缺血性脑卒中40例,FⅦ:C为97±2%,FⅦa为2.7±0.9ng/ml,两者高于对照组(P<0.05);Fbg测定各项值均增高(P<0.O5);纤维蛋白单体聚合反应速度(FMPV)为0.65±0.21,最大吸光度(Amax)为0.310±0.081,纤维蛋白原浓度(S)为3.80±1.65g/L,FMPV/Amax比值为2.097;反应延滞时间(DT)为13±3 s。结论:缺血性脑卒中患者血浆中FⅦa水平上调,FⅦ:C活性增高。Fbg不仅含量增加,其功能指数(FMPV/Amax)亦上调,表明患者存在明显的高凝状态。
Objective: To study the plasma levels of coagulation factor Ⅶ (FⅦ) and fibrinogen in patients with ischemic stroke and to elucidate its clinical significance. Methods: FⅦ assay: FⅦ antigen (FⅦ: Ag) by ELISA, FⅦ: C by one-stage coagulation method, activated FⅦa by recombinant soluble tissue factor (r-STF method). Fibrinogen (Fbg) determination: Acutobin hydrolysis - microcomputer automatic detection method. Results: There were 40 cases of ischemic stroke with FⅦ: C of 97 ± 2% and FⅦa of 2.7 ± 0.9ng / ml, both of which were higher than those of the control group (P <0.05) 0.50). Conclusion: FⅦa levels in plasma of patients with ischemic stroke are up-regulated and FⅦ: C activity is increased.
