目的 :研究同种异体血管移植在建立透析通路中的应用。方法 :将经冷冻、干燥和辐射处理后的人尸血管移植于 52例尿毒症患者前臂和上臂 ,建立永久性血管透析通路。结果 :手术成功率和术后 3~ 4周移植动静脉内瘘使用率均为 1 0 0 % ;至今 44例仍保持通畅 ,其中 2~ 3年 9例 ,1~ 2年 1 9例 ,不足 1年 1 6例 ,8例移植血管已失去功能 ,主要原因为血栓形成 5例 ,发生动脉瘤 2例 ,感染 1例。结论 :冻干辐射血管取材方便 ,价格低廉 ,容易保存 ,无抗原性 ,用于移植动静脉内瘘手术简单 ,成功率高 ,为自身动静脉内瘘建立困难的维持性血液透析患者提供了理想的移植血管通路。
Objective: To study the application of allogeneic vascular grafts in the establishment of dialysis access. Methods: Frozen, dried and irradiated human cadaver blood vessels were transplanted into the forearm and upper arm of 52 uremic patients to establish a permanent vascular dialysis access. Results: The success rate of operation and the rate of arteriovenous fistula transplantation were all 100% after 3 ~ 4 weeks of operation. So far, 44 cases remained patency, of which 9 cases were 2-3 years and 19 cases 1-2 years One year and 16 cases, 8 cases of implanted vessels have lost their function, mainly due to thrombosis in 5 cases, 2 cases of aneurysm occurred, 1 case of infection. Conclusion: The lyophilized irradiated blood vessels are easy to obtain, inexpensive, easy to store and antigen-free. They are simple and successful in transplanting arterio-venous fistula and provide an ideal for establishing difficult hemodialysis patients with arteriovenous fistula Transplantation of vascular access.
