目的应用肺保存液Euro-colins(EC液),观察不同肺动脉灌注压力对兔离体肺脏保存质量的影响。方法根据灌注压力1,2,3,4kPa的不同将兔分为G1、G2、G3、G4组,每组10只,按70ml/kg的总量经肺动脉灌EC液,根据压力调节流量,灌洗后肺脏置4℃保存液中冷藏4h后予新鲜静脉血进行复灌,测定血气分析、肺阻力、气道压力、肺组织的湿/干重比例及病理切片光镜检查。结果发现G2、G3的上述各项指标优于G1、G4组,G2、G3血氧满意,肺阻力降低,而G1组肺阻力明显增加,病理切片肺水肿明显,G4则肺泡出血明显。结论EC液对兔肺脏保存其最佳肺动脉的灌注压力为2~3kPa之间。
Objective To evaluate the effect of pulmonary artery perfusion pressure on the preservation quality of isolated lungs in vitro by using Euro-colins (EC fluid). Methods The rabbits were divided into G1, G2, G3 and G4 groups according to the different perfusion pressures of 1, 2, 3 and 4kPa. Each group consisted of 10 rabbits. After washing, the lungs were stored at 4 ℃ for 4h and re-filled with fresh venous blood. Blood gas analysis, pulmonary resistance, airway pressure, lung wet / dry weight ratio and pathological examination were determined by light microscopy. The results showed that G2 and G3 were superior to those of G1, G4, G2 and G3, and pulmonary resistance was decreased. However, lung resistance of group G1 was significantly increased, pulmonary edema was obvious in pathological section, and alveolar hemorrhage was obvious in G4. CONCLUSION EC perfusion pressure of 2 ~ 3kPa for rabbit pulmonary preservation of its optimal pulmonary artery.
