目的探讨不同分娩方式对产妇初乳分泌及初乳中免疫球蛋白A(sIgA)含量的影响,寻找降低剖宫产率的预防对策。方法选择自然分娩(Ⅰ组)、剖宫产(Ⅱ组)及剖宫产术后使用吗啡(Ⅲ组)的初产妇各40例,记录分娩后首次产生初乳的时间,测定泌乳量和测定首次产生初乳及初乳产生后24、48 h初乳中sIgA含量。结果产生首次初乳的时间顺序为Ⅰ<Ⅲ<Ⅱ(P均<0.01);初乳中sIgA值:首次初乳Ⅰ>Ⅲ>Ⅱ(P均<0.01);初乳产生后24 hⅡ<Ⅰ及Ⅲ(P均<0.05);初乳产生后48 h三组含量差异无统计学意义(P>0.05)。结论自然分娩有利于产妇缩短初乳分泌时间,增加分泌乳量及初乳中sIgA含量。因此,提倡自然阴道分娩。
Objective To investigate the effects of different modes of delivery on the secretion of colostrum and the content of immunoglobulin A (sIgA) in colostrum, and to find out the preventive measures to reduce the rate of cesarean section. Methods 40 cases of primipara of spontaneous labor (group Ⅰ), cesarean section (group Ⅱ) and morphine (Ⅲ group) after cesarean section were selected. The time of the first colostrum after delivery was recorded, and the amount of milk was measured. SIgA content of colostrums at 24 and 48 h after colostrum and colostrum were first produced. Results The time sequence of the first colostrum was Ⅰ <Ⅲ <Ⅱ (all P <0.01). The sIgA of colostrum: Ⅰ> Ⅲ> Ⅱ (all P <0.01) And Ⅲ (P <0.05). There was no significant difference in the content of the three groups after 48 h of colostrum formation (P> 0.05). Conclusion Natural delivery is beneficial to maternal shortening the secretion of colostrum, increasing the amount of milk produced and the content of sIgA in colostrum. Therefore, to promote natural vaginal delivery.
