目的分析孕早期妇女弓形虫(TOX)不同感染状况对出生缺陷的影响。方法用酶联免疫吸附法检测孕早期妇女血中弓形虫IgG及IgM抗体,根据检验结果组成急性感染组、活动性感染组、既往感染组,随机抽取无感染者作对照组,比较4组妊娠结局。结果在3 500名孕早期妇女中弓形虫感染率为7.69%,其中近期急性感染占16.9%,活动性感染占30.1%,既往感染占53.6%。比较4组结果为:各组出生缺陷率差别有统计学意义(P<0.05),其中急性感染组出生缺陷率高于活动性感染组、既往感染和未感染组(P<0.05),活动性感染组高于既往感染和未感染组(P<0.05),既往感染和未感染组之间差别无统计学意义(P>0.05)。结论孕早期Tox既往感染者出生缺陷的发生与其是否发生再次感染或激发感染复发密切相关。孕早期Tox不同感染状况出生缺陷发生率从大到小次序为:急性感染、活动性感染、既往感染和无感染。
Objective To analyze the effect of different infection status of TOX on the birth defects in the first trimester of pregnancy. Methods Toxoplasma gondii IgG and IgM antibodies in pregnant women were detected by enzyme-linked immunosorbent assay (ELISA). According to the test results, the acute infection group, the active infection group and the previous infection group were randomly selected as the control group. ending. Results The prevalence of Toxoplasma in 3 500 pregnant women was 7.69%. Among them, acute infection accounted for 16.9%, active infection accounted for 30.1% and previous infection accounted for 53.6%. Comparing the results of 4 groups, the difference of birth defects in each group was statistically significant (P <0.05). The incidence of birth defects in acute infection group was higher than that in active infection group, previous infection group and non-infected group (P <0.05) The infection group was higher than those of the former infection group and the uninfected group (P <0.05). There was no significant difference between the former infected group and the uninfected group (P> 0.05). Conclusion The incidence of birth defects in the first trimester Tox in early pregnancy is closely related to whether there is re-infection or the recurrence of infection. In early pregnancy, the incidence of birth defects in different infectious status of Tox in descending order: acute infection, active infection, previous infection and no infection.
