传统认为,栓塞很少引起大脑后循环卒中,这种说法是毫无根据的。我们查阅了已证实的椎基底动脉各部位动脉闭塞病例的有关资料,试图查明潜在的栓塞易发部位及栓子的来源,并深入地研究了100余例脑干及小脑梗塞患者的卒中发病机理。结果表明:(1)心原性和动脉原性栓塞各占大脑后循环梗塞的20%;(2)最常见的动脉原性栓子是椎动脉的起始部和颅内段的粥样狭窄病变;(3)椎动脉剥离、颈部和脊椎创伤、手术中颈部长时处于不自然的位置也可引起椎动脉血栓性栓塞;(4)最常发生栓塞的部位是基底动脉远端、小脑上动脉、大脑后动脉区、椎动脉颅内段及小脑后下动脉区。
Traditionally, the idea that embolization rarely causes a stroke in the back of the brain is entirely unfounded. We reviewed available data on arterial occlusion of vertebrobasilar arteries and sought to identify potential sources of embolic risk and emboli, as well as in-depth studies of stroke in more than 100 patients with brain stem and cerebellar infarcts mechanism.
