目的 :检测类风湿关节炎 (RA)患者血清中人及猪Ⅱ型胶原抗体水平 ,探讨其在RA发病中的意义。方法 :用自提取的人、猪Ⅱ型胶原包板 ,采用改良的酶联免疫吸附法 (ELISA)测定 1 0 5例RA患者血清抗人、猪Ⅱ型胶原抗体IgG及IgA ,并与正常人 ( 1 4 0例 )及疾病对照组 ( 80例 )比较分析。同时检测患者的类风湿因子(RF)、血沉 (ESR)及C反应蛋白 (CRP)。结果 :RA患者血清中抗人Ⅱ型胶原抗体IgG、IgA阳性率分别为2 5 .3%、2 7.5 % ,抗猪Ⅱ型胶原抗体IgG、IgA阳性率分别为 2 6.9%、2 5 .6% ,显著高于正常人及疾病对照组 (P<0 .0 5 )。两种属Ⅱ型胶原抗体IgG及IgA常同时存在于同一患者中。两种属Ⅱ型胶原抗体与RF、ESR及CRP异常无相关性。结论 :RA患者血清中人及猪Ⅱ型胶原抗体水平升高 ,可能在RA的发病中起着一定的作用 ;但RA患者Ⅱ型胶原抗体阳性与疾病活动指标无明显相关 ,不能作为监测RA活动的指标。
Objective: To detect the serum level of human and porcine type Ⅱ collagen in patients with rheumatoid arthritis (RA) and to explore its significance in the pathogenesis of RA. Methods: Serum anti-human and porcine type Ⅱ collagen IgG and IgA of 105 RA patients were detected by modified enzyme-linked immunosorbent assay (ELISA) (140 cases) and disease control group (80 cases). Patients were also tested for rheumatoid factor (RF), ESR and CRP. Results: The positive rates of IgG and IgA of anti-human type II collagen in serum of RA patients were 25.3% and 25.5%, respectively. The positive rates of IgG and IgA of anti-porcine type II collagen antibody were respectively 2 6.9% and 25.6 %, Significantly higher than normal and disease control group (P <0.05). Both genus type II collagen IgG and IgA often co-exist in the same patient. There was no correlation between the two genotypes of collagen antibody and the abnormalities of RF, ESR and CRP. CONCLUSIONS: The elevated levels of human and porcine type II collagen antibodies in serum of RA patients may play a role in the pathogenesis of RA. However, there is no significant correlation between the type II collagen antibody positive and disease activity index in RA patients, index of.
