目的 了解佛山市顺德区2014-2015年健康人群白喉抗体水平.方法 采取分层随机抽样方法, 在全区10个镇/街道选择0-63岁健康人群采集血清标本, 采用酶联免疫吸附试验 (ELISA) 检测白喉IgG抗体, 分析白喉抗体水平.结果 调查人群白喉IgG抗体阳性率为73.30% (324/442), 抗体几何平均浓度 (GMC) 为0.32IU/mL.本地、外地户籍人群抗体阳性率分别为79.05% (234/296) 、61.64% (90/146) (χ2=15.14, P<0.01) .抗体阳性率在4岁 (94.12%) 、8或9岁 (100%) 达到高峰, 随后逐渐降至21-30岁的53.57%、≥31岁的38.24%.含白喉成分疫苗接种剂次越多, 阳性率越高 (趋势χ2=72.51, P<0.01);接种年限越长, 阳性率越低 (趋势χ2=4.53, P=0.03) .结论 顺德区健康人群已形成有效的白喉免疫屏障.建议>20岁往来于白喉流行国家或地区人群定期加强免疫1剂次含白喉成分疫苗.“,”Objective To evaluate diphtheria antibody levels of healthy people in Shunde district of Foshan city during2014-2015. Methods We used stratified random sampling to obtain serum samples f rom 0-63 year old healthy people in10 townships of Shunde. We tested sera for diphtheria IgG antibody by enzyme-linked immunosorbent assay (ELISA) and analyzed diphtheria antibody levels. Results The positivity rate and geometric mean concentration (GMC) of diphtheria IgG antibody among subjects were 73.30% (324/442) and 0.32 IU/mL, respectively. Positivity rates among local and non-local subjects were 79.05% (234/296) and 61.64% (90/146), respectively (χ2=15.14, P<0.01). Positivity rates peaked at 4 years of age (94.12%), and 8 or 9 years of age (100%), and then gradually decreased to 53.57%among 21-30 year olds and 38.24% among ≥31 year olds. Receiving more doses of diphtheria containing vaccine was associated with higher sero-positivity (trend χ2=72.51, P<0.01). Longer time since vaccination was associated with lower sero-positivity (trend χ2=4.53, P=0.03). Conclusions An effective immunity barrier against diphtheria has been formed among healthy people in Shunde. We recommend 1 booster dose of diphtheria containing vaccine for>20-yearold people travelling to diphtheria-endemic countries or regions.
