中医处方是医师为患者医疗或预防需要而开写给药房的书面文件,它直接关系到医疗效果。但近年来从大量的处方中,我们常常可以见到一些不规范的现象。其原因:一是我国中药来源复杂,商品中药又存在着同名异物、同物异名、伪品、类同品、代用品等问题;二是由于部分医疗人员对药材来源不十分清楚,因此在处方时常出脱各种问题,又使中药更加混乱。这样会给药房工作带来一定困难,容易出现误差,以致影响临床效果。归纳起来主要有以下几种情况。
