经皮穿刺冠状动脉腔内成形术后左室功能的改善

有关经皮穿刺冠状动脉腔内成形术的可行性和短、中期疗效已经明确。一些作者报道患者术后在平静状态时,心肌对铊的摄取改善,运动时改善更明显,冠状动脉血流增加,心肌乳酸生成减少,临床症状改善,非侵入性左室功能参数正常化。但此新疗法对左室工作的效益尚未完全肯定,因而本文对有严重冠状动脉病变患者在成功地施行导管扩张术的前、后,采用侵入和非侵人性方法测量其左室功能,以观察术后左室功能改善的情况。方法:男性患者7例,经冠状动脉造影证实左前降支近端高度或接近完全阻塞,心绞痛程度Ⅲ~Ⅳ级,运动时 ST-T 发生可逆性改变而无心肌梗塞史。检查前一日开始停用硝酸甘油长效制剂和β-阻滞剂,在安静和平卧运动(82±17瓦)下用~99锝标记的人体白蛋白作首次通过和门电路血池扫描。然后将 Swan-Ganz 7F 双腔热敏电阻导管置入肺动脉,Millar PC 771(7F)型双端微型压力计 The feasibility and short, medium-term efficacy of percutaneous transluminal coronary angioplasty has been clear.
