目的探讨脑干听觉诱发电位(BAEP)对糖尿病神经病变患者听神经中枢段受累情况的检测价值。方法用丹麦产DANTEC的KEYPOINT 4th诱发电位仪对48例糖尿病神经病变患者组及年龄和性别与之匹配的2型糖尿病(DM)且临床检查未发现周围神经病变的患者组进行BAEP检测。结果糖尿病神经病变组BAEP波Ⅲ潜伏期(3.7±0.21)ms与无神经系统症状的DM患者组(3.4±0.18)ms比较差异有统计学意义(P<0.01);Ⅰ~Ⅲ峰间潜伏期分别为(2.9±0.18)和(2.6±0.14)ms,P<0.01及Ⅲ~Ⅴ峰间潜伏期(2.12±0.24)和(1.97±0.19)ms,P<0.01。结论BAEP检测对判断糖尿病神经病变患者听神经中枢段是否受累及受累部位有意义。
Objective To explore the value of brainstem auditory evoked potentials (BAEP) in the auditory nerve central involvement in patients with diabetic neuropathy. Methods BAEP was performed in 48 patients with diabetic neuropathy and with age-and-gender-matched type 2 diabetes mellitus (DM) in the DANTEC KEYPOINT 4th Evoked Potentiometer in Denmark and with no neuropathies in clinical examination. Results The latency of BAEP wave Ⅲ in diabetic neuropathy group was (3.7 ± 0.21) ms vs that in DM patients without neurological symptoms (3.4 ± 0.18) ms (P <0.01). The incubation period of Ⅰ ~ Ⅲ peak in diabetic neuropathy group was (2.9 ± 0.18) and (2.6 ± 0.14) ms respectively, P <0.01 and latency between Ⅲ ~ Ⅴ (2.12 ± 0.24) and (1.97 ± 0.19) ms, P <0.01. Conclusion The detection of BAEP is of significance in judging whether central nervous system involvement and involvement of auditory nerve in patients with diabetic neuropathy are involved.
