目的研究并探讨低剂量螺旋CT对筛选检查早期肺癌的临床价值。方法以我院收治的400例体检患者为研究对象,对所有患者进行普通CT以及低剂量螺旋CT进行前瞻性分析。对患者进行筛选检查的低剂量螺旋CT为Philip SR7000以及GELightSpeed Pius多层CT扫描机,扫描出来的图像由两位放射医师在其工作站的显示器以电影显示的方式进行详细的观察与分析,并将其存储在影响及传输系统中。结果低剂量螺旋CT检测出100例非钙化结节,普通CT检测出15例,所有患者均为一期病变。低剂量螺旋CT共检测出叶支气管或段支气管病变患者8例,其中早期中央型肺癌患者3例,普通CT均未检测出来。普通CT以及低剂量螺旋CT对于早期肺癌的敏感率分别为44.5%和100%,特异度分别为88%和80%。结论通过对初步的筛选结果进行较为科学与全面的分析得知低剂量螺旋CT显著提高了对患者肺内非钙化小结以及支气管细微病变的检测,其对于早期肺癌的检测具有显著的积极意义,值得在临床中进行大力的推广与应用。
Objective To study and discuss the clinical value of low-dose spiral CT in the screening of early stage lung cancer. Methods A total of 400 medical examinations admitted to our hospital were studied. All patients underwent routine CT and low-dose spiral CT for prospective analysis. The low-dose spiral computed tomography screening of patients was performed on the Philips SR7000 and GELightSpeed Pius multi-slice CT scanners. The scanned images were viewed and analyzed in cinematographic form by two radiologists on their workstations’ monitors. It is stored in the impact and transmission system. Results Low-dose spiral CT detected 100 cases of non-calcified nodules, common CT detected 15 cases, all patients were a disease. Low-dose spiral CT detected bronchial or segmental bronchial lesions in 8 patients, including 3 patients with early-stage central lung cancer, CT were not detected. The sensitivity rates of common CT and low-dose spiral CT for early lung cancer were 44.5% and 100%, respectively, and the specificity were 88% and 80% respectively. Conclusion Through a more scientific and comprehensive analysis of the preliminary screening results, we know that low-dose spiral CT can significantly improve the detection of non-calcified nodules and bronchial micro-lesions in the lungs, which is of great significance for the detection of early lung cancer.
