目的探讨2型糖尿病患者血清基质金属蛋白酶-8(MMP-8)水平与颈动脉粥样硬化的相关性。方法选择正常对照组(NC组)60例、糖尿病组(DM组)89例。糖尿病组根据颈动脉内-中膜厚度(IMT)及斑块情况分为糖尿病非颈动脉粥样硬化组(DM1组,n=16)和糖尿病合并颈动脉粥样硬化组(DM2组,n=73),比较三组的血清MMP-8水平;DM2组分为IMT增厚组(A组,n=12)、稳定斑块组(B组,n=27)和不稳定斑块组(C组,n=34),比较三个亚组的IMT、Crouse积分等超声指标,分析MMP-8水平与颈动脉超声指标的相关性。结果DM2组的MMP-8、IMT值水平高于NC组和DM1组,DM1组高于NC组(P均<0.05);不稳定斑块组的MMP-8水平高于稳定斑块组和IMT增厚组,稳定斑块组MMP-8水平高于IMT增厚组(P均<0.05);血清MMP-8水平与IMT值和Crouse积分呈正相关(r值分别为0.68、0.72,P<0.05)。结论血清MMP-8与2型糖尿病动脉粥样硬化斑块的形成和斑块稳定性相关,可作为预测颈动脉斑块形成及稳定与否的血清学指标。
Objective To investigate the relationship between serum matrix metalloproteinase-8 (MMP-8) and carotid atherosclerosis in type 2 diabetic patients. Methods Sixty normal controls (NC group) and 89 diabetic patients (DM group) were selected. According to carotid intima-media thickness (IMT) and plaque status, diabetic patients were divided into diabetes non-carotid atherosclerosis group (DM1 group, n = 16) and diabetes mellitus with atherosclerosis group (DM2 group, n = 73). The levels of serum MMP-8 in the three groups were compared. The DM2 group consisted of IMT thickened group (group A, n = 12), stable plaque group (group B, n = 27) and unstable plaque group Group, n = 34). The IMT and Crouse integral were compared among the three subgroups. The correlation between MMP-8 level and carotid artery ultrasonography was analyzed. Results The levels of MMP-8 and IMT in DM2 group were higher than those in NC group and DM1 group, and DM1 group was higher than that in NC group (all P <0.05). MMP-8 in unstable plaque group was higher than that in stable plaque group and IMT The levels of MMP-8 in thickened group and stable plaque group were higher than those in IMT group (all P <0.05). The serum MMP-8 level was positively correlated with IMT value and Crouse score (r = 0.68,0.72, P <0.05 ). Conclusions Serum MMP-8 is associated with the formation of atherosclerotic plaque in type 2 diabetes mellitus and the stability of plaque. It may be used as a serological marker to predict the formation and stability of carotid plaque.
