目的观察阿奇霉素序贯疗法治疗小儿支原体肺炎的临床效果。方法选择2015年1月-2017年3月收治的小儿支原体肺炎患儿90例,按随机数字化原则分为观察组和对照组各45例,2组均给予吸氧、解痉、平喘、降温等治疗,对照组应用阿奇霉素持续静脉滴注治疗,观察组应用阿奇霉素序贯疗法。治疗2周后观察2组患儿症状、体征消失时间,比较2组阿奇霉素静脉滴注时间、临床疗效及不良反应(静脉炎、恶心、呕吐、腹泻、腹痛、肝功能损害)。结果 2组热退时间、咳止时间、肺啰音消失时间比较差异均无统计学意义(P>0.05);观察组阿奇霉素静脉滴注时短于对照组,差异有统计学意义(P<0.05)。观察组总有效率为95.56%,对照组总有效率为97.78%,2组比较差异无统计学意义(χ2=0.22,P>0.05)。观察组不良反应发生率为22.22%(10/45)低于对照组的60.00%(27/45),差异有统计学意义(χ2=9.54,P<0.05)。结论阿奇霉素序贯疗法治疗小儿支原体肺炎疗效确切,不良反应少,方便易行,有临床推广价值。
Objective To observe the clinical effect of sequential treatment of azithromycin in children with mycoplasma pneumonia. Methods Ninety children with mycoplasma pneumonia admitted from January 2015 to March 2017 were divided into observation group (45 cases) and control group (45 cases) according to the principle of random digitization. Both groups were given oxygen, antispasmodic, antiasthmatic and cooling And other treatment, the control group with azithromycin continuous intravenous infusion of the observation group applied azithromycin sequential therapy. After 2 weeks of treatment, the symptoms and signs disappeared in 2 groups. The time of intravenous infusion of azithromycin, clinical efficacy and adverse reactions (phlebitis, nausea, vomiting, diarrhea, abdominal pain and liver damage) were compared between the two groups. Results There was no significant difference between the two groups in the thermal back-off time, the cough stop time and the disappearance time of pulmonary rales (P> 0.05). The azithromycin in the observation group was shorter than the control group by intravenous drip, the difference was statistically significant (P <0.05 ). The total effective rate was 95.56% in the observation group and 97.78% in the control group. There was no significant difference between the two groups (χ2 = 0.22, P> 0.05). The incidence of adverse reactions in the observation group was 22.22% (10/45), which was lower than that in the control group (60.00%, 27/45). The difference was statistically significant (χ2 = 9.54, P <0.05). Conclusion Sequential azithromycin treatment of children with mycoplasma pneumonia curative effect is accurate, less adverse reactions, convenient and easy to have clinical value.
