患者女,61岁。因间断性腹胀,隐痛10年,腹部胀大,皮肤瘙痒20天,于1993年4月11日入院。20年前患“肺结核”,无肝炎病史。 体查:体温37℃,血压12/8KPa。全身皮肤无黄染、出血点及蜘蛛痣,双侧颈部淋巴结如蚕豆大小,有粘连,无触痛。心肺(-)。腹膨隆,腹围83cm,腹壁静脉无曲张,无压痛及包块,肝脾未及腹水征(+)。双足踝凹陷性水肿。辅助检查:血常规:血红蛋白110g/L,白细胞8.6×10~9/L,中性80％,淋巴20％。血小板120×10~9/L,血沉7mm/h。肝功:总胆红素22μmol/L,直接胆红素10umol/L,谷丙转氨酶10~(?)(赖氏法),HBSAg(-),总蛋白54g/L,白蛋白
Female patient, 61 years old. Due to intermittent bloating, pain for 10 years, abdominal swelling, skin itching 20 days, in April 11, 1993 admission. 20 years ago suffering from “tuberculosis”, no history of hepatitis. Physical examination: body temperature 37 ℃, blood pressure 12 / 8KPa. No skin yellow blemishes, bleeding points and spider nevus, bilateral cervical lymph nodes such as broad bean size, adhesions, no tenderness. Cardiopulmonary (-). Abdominal bulging, abdominal circumference 83cm, abdominal varicose veins, no tenderness and mass, liver and spleen and ascites (+). Sagittal ankle depression edema. Auxiliary examination: blood: hemoglobin 110g / L, white blood cells 8.6 × 10 ~ 9 / L, 80% neutral, lymphatic 20%. Platelets 120 × 10 ~ 9 / L, ESR 7mm / h. Liver function: total bilirubin 22μmol / L, direct bilirubin 10umol / L, alanine aminotransferase 10 ~ (?), HBSAg (-), total protein 54g / L, albumin
