目的探讨三维CT血管成像(3D-CTA)与DSA(数字减影血管造影检查)检查诊断颅内动脉瘤的对比情况。方法对40例动脉瘤患者经3D-CTA与DSA检查出的46个动脉瘤予以比较研究,评估出3D-CTA对颅内动脉瘤的诊断价值。结果40例患者的46个动脉瘤,3D-CTA准确检出42个,DSA检出45个,两者检出率差异无统计学意义(P>0.05),但是在小动脉瘤的诊断上3D-CTA不及DSA,而3D-CTA在载瘤动脉的显示、瘤壁钙化及瘤周解剖标志等方面,明显优于DSA(P<0.05)。结论3D-CTA可以作为颅内动脉瘤诊断和治疗的首选。
Objective To investigate the contrast between three dimensional CT angiography (3D-CTA) and DSA (digital subtraction angiography) in diagnosis of intracranial aneurysms. Methods Forty-four aneurysms of 40 aneurysms were studied by 3D-CTA and DSA. The diagnostic value of 3D-CTA for intracranial aneurysms was evaluated. Results Forty-four aneurysms were detected in 40 patients, including 42 accurately detected by 3D-CTA and 45 detected by DSA. There was no significant difference between the two methods (P> 0.05), but in the diagnosis of small aneurysms -CTA was inferior to DSA, while 3D-CTA was significantly better than DSA in the display of host artery, tumor wall calcification and tumor peritumoral sign (P <0.05). Conclusion 3D-CTA can be the first choice for diagnosis and treatment of intracranial aneurysms.
