目的 :检测双肾上腺素样激酶1(doublecortin-like kinase 1,DCLK1)在结直肠癌患者血浆中的表达水平,并通过与癌胚抗原(carcinoembryonic antigen,CEA)的对比,探讨其诊断价值。方法 :收集78例结直肠癌患者手术前的血浆标本和48例健康志愿者的血浆标本,应用酶联免疫吸附试验(enzyme-linked immunosorbent assay,ELISA)检测DCLK1和CEA的表达水平,对比分析DCLK1、CEA及二者联合检测诊断结直肠癌的感敏度、特异度和准确性,并分析血浆中DCLK1和CEA的表达水平与结直肠癌患者临床病理特征之间的关系。结果 :DCLK1和CEA在结直肠癌患者血浆中的表达水平均明显高于健康志愿者(P<0.01)。DCLK1诊断结直肠癌的敏感度和准确性均优于CEA,DCLK1与CEA联合检测可进一步提高诊断结直肠癌的敏感度和准确性。血浆中DCLK1的含量与结直肠癌患者的TNM分期、分化程度、有无淋巴结转移及有无脉管癌栓等相关(P<0.01)。结论 :血浆DCLK1有望成为诊断结直肠癌的一种新型肿瘤标志物,且与结直肠癌患者的恶性程度密切相关。
OBJECTIVE: To detect the expression of double-labeled kinase 1 (DCLK1) in the plasma of patients with colorectal cancer and to explore its diagnostic value by comparing with carcinoembryonic antigen (CEA). Methods: Plasma samples from 78 preoperative colorectal cancer patients and 48 healthy volunteers were collected. The expression of DCLK1 and CEA were detected by enzyme-linked immunosorbent assay (ELISA), and the expression of DCLK1 , CEA and their combined detection of colorectal cancer sensitivity and specificity and accuracy, and analysis of plasma DCLK1 and CEA expression levels and clinicopathological features of patients with colorectal cancer. Results: The expression levels of DCLK1 and CEA in plasma of patients with colorectal cancer were significantly higher than those of healthy volunteers (P <0.01). The sensitivity and accuracy of DCLK1 in the diagnosis of colorectal cancer were better than that of CEA. The combined detection of DCLK1 and CEA could further improve the sensitivity and accuracy of diagnosis of colorectal cancer. The content of DCLK1 in plasma was correlated with TNM stage, differentiation degree, lymph node metastasis and presence or absence of vascular tumor thrombus in patients with colorectal cancer (P <0.01). Conclusion: Plasma DCLK1 is expected to be a novel tumor marker for diagnosing colorectal cancer, which is closely related to the malignancy of patients with colorectal cancer.
