目的:提高神经外科病房临床护理交接班质量和效率。方法:浙江省立同德医院于2014年1月开始时实施SBAR沟通模式,根据SBAR沟通模式内容,神经外科结合科室自身的特点设计制定病人交接核查表,对科室护理人员进行相关SBAR沟通模式的培训,进一步规范科室的交接班流程。比较SBAR沟通模式实施前后1年两组患者在交接班过程中交接班问题的发生率与医护人员对SBAR沟通模式的满意程度。结果:2013年共审核交接班患者400例,其中发生交接班问题82例,2014年共审核交接班患者450例,其中发生交接班问题38例,SBAR沟通模式实施后交接班问题的发生明显较实施前减少(P<0.05)。交接班时间较实施前有明显的缩短,医护人员对交接班的质量和效率的满意程度有明显的提高(P<0.05)。结论:在神经外科护理交接班过程中实施SBAR沟通模式,可以有效加强医护团队的合作,提高医护人员交接班的质量和效率,减少了交接班过程中安全隐患事件的发生,有效保证患者的安全。
OBJECTIVE: To improve the quality and efficiency of clinical nurse handovers in neurosurgical wards. Methods: The SBAR communication mode was implemented at the beginning of January 2014 in Tongde Hospital of Zhejiang Province. Based on the content of SBAR communication mode and the characteristics of department of neurosurgery combined with the department itself, a checklist of patient handover was made and the relevant SBAR communication mode Training, to further standardize the section of the shift process. The SBAR communication pattern was compared between the incidence of shift problems and the satisfaction level of SBAR communication mode between two groups before and after 1 year of SBAR communication. Results: In 2013, a total of 400 transfer patients were reviewed, of whom 82 were involved in the transfer. In 2014, a total of 450 transfer patients were reviewed, of whom 38 were involved in the handover. The occurrence of handover problems after the implementation of the SBAR communication mode was significantly worse Before implementation decreased (P <0.05). The handover time was significantly shorter than that before implementation, and the satisfaction of medical staff on the quality and efficiency of the handover was significantly improved (P <0.05). Conclusion: The implementation of SBAR communication mode during the delivery of neurosurgery nursing care can effectively enhance the cooperation between medical teams, improve the quality and efficiency of hand-over of medical staff, reduce the occurrence of safety hidden trouble during delivery and effectively ensure the safety of patients .
