目的动态观察新生儿缺氧缺血性脑损伤患儿血清中IGF-1水平的变化,探讨血清IGF-1在新生儿缺氧缺血性脑损伤时病情的早期诊断、程度判断的意义。方法选择66例新生儿缺氧缺血性脑病患儿作为观察对象,并根据病情轻重分为轻度、中度、重度,以16例健康新生儿作为对照组。检测新生儿出生第1,3,7天血清中IGF-1的水平变化。结果HIE患儿血清IGF-1在出生第1,3,7天均显著下降,与对照组比较差异有统计学意义(P<0.01),24h内达低峰,以后逐渐上升;3种程度HIBD在其检测时间点上IGF-1较对照组差异均有统计学意义(P<0.05);重度至第7天时尚不能达到正常水平。结论IGF-1变化对判断有无HIBD及其严重程度有重要意义,可作为判断临床病情轻重程度的指标之一。
Objective To observe the changes of serum IGF-1 levels in neonates with hypoxic-ischemic brain damage and to explore the significance of serum IGF-1 in the early diagnosis and degree judgment of neonatal hypoxic-ischemic brain damage. Methods Sixty-six neonates with hypoxic-ischemic encephalopathy were selected as observation subjects and divided into mild, moderate and severe according to their severity. Sixteen healthy newborns were selected as control group. Serum levels of IGF-1 on the 1st, 3rd and 7th day after birth were detected. Results The serum levels of IGF-1 in HIE children decreased significantly on the 1st, 3rd, 7th day of birth, compared with the control group (P <0.01), reached the peak value in 24h and then gradually increased. The levels of HIBD At the time point of detection, the difference between IGF-1 and control group was statistically significant (P <0.05); severe to the seventh day can not reach the normal level. Conclusion The change of IGF-1 is of great significance for judging whether HIBD is serious or not, and it can be used as one of the indexes to judge the severity of clinical condition.
