目的 探讨不同类型急性脑卒中者中无症状性脑梗死 (ACI)的发病率、危险因素、MRI特征及可能的临床意义。方法 本研究包括 83 6例因急性脑卒中在我院就诊并进行MRI检查的患者 ,对无症状脑梗死的MRI特征、危险因素、以及在各组脑卒中者中的分布情况加以分析。结果 83 6例中 ,3 1.8%有无症状性脑梗死 ,占脑梗死组的 4 9.4 %、腔隙性梗死组的 3 3 .8%、TIA组的 15 .0 %、脑出血组的 2 0 .6%。ACI主要为腔隙性脑梗死 ,分布于内囊、基底节、丘脑及半卵圆中心。逻辑回归分析表明 ,缺血性心脏病、高血压、年龄及低血压是其独立危险因素。结论 ACI主要以腔隙性为主、并与缺血性卒中呈正相关 ,其危险因素与缺血性卒中相似
Objective To investigate the incidence, risk factors, MRI features and possible clinical significance of asymptomatic cerebral infarction (ACI) in different types of acute stroke. METHODS: This study included 83 6 patients with acute stroke who were seen in our hospital and undergoing MRI. The MRI features of asymptomatic cerebral infarction, risk factors, and the distribution among stroke patients in each group were analyzed. Results Among 83 6 cases, 3 1.8% had asymptomatic cerebral infarction, accounting for 4 9.4% of cerebral infarction group, 33.8% of lacunar infarction group, 15.0% of TIA group and 2 of cerebral hemorrhage group 0 .6%. ACI is mainly lacunar infarction, located in the internal capsule, basal ganglia, thalamus and semi-oval center. Logistic regression analysis showed that ischemic heart disease, hypertension, age and hypotension were independent risk factors. Conclusions ACI is predominately lacunar and has a positive correlation with ischemic stroke with a similar risk factor to ischemic stroke
