目的探讨磁敏感加权成像(Susceptibility-Weighted Imaging,SWI)在四肢软组织海绵状血管瘤的诊断价值。方法回顾性分析50例临床及病理证实的四肢软组织海绵状血管瘤患者的影像资料,对比SWI序列与常规扫描序列(包括快速自旋回波序列TSE T1WI及T2WI;液体衰减反转恢复序列T2 FLAIR;脂肪抑制SE T2WI)的影像诊断。结果 50例共检出62个病灶,8例为多发。SWI结合常规序列共显示Ⅰ(结节)型9个病灶,Ⅱ(局限团块)型17个,Ⅲ(弥漫蔓藤)型25个和Ⅳ(混合)型11个。其中24个病灶为低回流型病灶;38个为高回流型。结论 SWI序列作为常规MRI序列的重要补充序列,可以更全面、准确的反映四肢软组织海绵状血管瘤内部构成成分,显示畸形血管,鉴别钙化和出血及铁沉积等,对于临床诊断、制定相关治疗方案及评估预后有着重要的应用价值。
Objective To investigate the diagnostic value of Susceptibility-Weighted Imaging (SWI) in soft tissue cavernous hemangiomas. Methods Fifty cases of cavernous hemangiomas with limbal soft tissue confirmed by clinical and pathological features were retrospectively analyzed. The SWI sequences and conventional scanning sequences (including the rapid spin echo sequence TSE T1WI and T2WI, the T2 FLAIR of liquid attenuation reversal recovery sequence, Fat suppression SE T2WI) imaging diagnosis. Results 50 cases were detected in 62 lesions, 8 cases of multiple. SWI combined with routine sequence showed 9 lesions of Ⅰ (nodular) type, 17 of Ⅱ (limited mass), 25 of Ⅲ (diffuse vine) and 11 of Ⅳ (mixed). Among them, 24 lesions were of low reflux type and 38 were of high reflux type. Conclusion SWI sequence as an important complement sequence of conventional MRI sequences can more completely and accurately reflect the internal components of cavernous hemangioma of soft tissue of the extremities, showing abnormal blood vessels, differentiating calcifications, hemorrhage and iron deposition. For the clinical diagnosis, the related treatment plans And assess the prognosis has important application value.
