[目的]探讨双侧甲状腺癌甲状腺全切+Ⅵ区淋巴结清扫术中对甲状旁腺的保护措施。[方法]回顾性分析67例双侧甲状腺癌患者的临床资料。[结果]67例患者均行甲状腺全切加双侧Ⅵ区淋巴结清扫术,术后6~48h内有15例(22.39%)患者出现暂时性甲状旁腺功能低下,无永久性甲状旁腺功能低下病例发生。[结论]甲状腺全切加双侧Ⅵ区淋巴结清扫术中应用超声刀进行甲状旁腺及其供应血管的原位保护可有效预防甲状旁腺功能低下的发生。
[Objective] To explore the protective measures of parathyroid gland in complete thyroidectomy + Ⅵ lymph node dissection in bilateral thyroid cancer. [Methods] The clinical data of 67 patients with bilateral thyroid carcinoma were retrospectively analyzed. [Results] All patients underwent total thyroidectomy plus bilateral Ⅵ lymph node dissection. Fifteen patients (22.39%) within 6 to 48 hours after operation had transient hypoparathyroidism and no permanent parathyroid function Low cases occur. [Conclusion] The application of ultrasonic scalpel to the parathyroid gland and its supplying blood vessel in situ in the total thyroidectomy plus bilateral Ⅵ lymph node dissection can effectively prevent the hypoparathyroidism.
