对已有相配供体的严重再生障碍性贫血(再障)病人,骨髓移植是可以选择的一种治疗方法。同种骨髓移植的主要障碍是移植物的排斥和移植物抗宿主病(GVHD)。作者发展了一种再障病人移植前免疫抑制的新方法,即全身淋巴照射与环磷酰胺联合应用的方案。此方案既得到了照射的免疫抑制作用,又免于全身照射所造成的疾患,并且亦降低了单纯使用环磷酰胺时所见到的排斥发生率。在1977~1979年间,作者对9名在移植前曾接受多次输血的严重再障病人进行了移植。年龄1~18岁,中位数11岁。
For patients with severe aplastic anemia (aplastic anemia) who have a matching donor, bone marrow transplantation is an alternative treatment. The main hurdle for allogeneic bone marrow transplantation is graft rejection and graft versus host disease (GVHD). The authors developed a new method of immunosuppression before transplantation in patients with aplastic anemia, which is the combination of systemic lymphatic irradiation with cyclophosphamide. This regimen not only achieves the immunosuppressive effects of irradiation, but also avoids the conditions caused by systemic irradiation and also reduces the incidence of rejection seen with cyclophosphamide alone. Between 1977 and 1979, the authors transplanted nine severely aplastic anemia patients who had undergone multiple transfusions before transplantation. Ages 1 to 18 years old, median 11 years old.
