Objective: To investigate the relationship between the operative timing and the prognosis of acute closed head injury and intracranial hematoma. Methods: A retrospective analysis of 2187 cases of acute closed head trauma and intracranial hematoma confirmed by CT and surgery from February 2005 to October 2015 in the department was conducted to explore the relationship between the timing of surgery and prognosis. Results: There were 1 805 cases (82.5%) recovered well, 258 cases (11.8%) with mild disability, 39 cases (1.8%) with severe disability and 243 cases (11.1%) with plant survival or death. Conclusion: In the case of acute closed craniocerebral trauma with intracranial hematoma, it is the best time to grasp the operation before coma or brain herniation, which can obviously improve the prognosis of patients.
