目的分析双向转诊管理对糖尿病患者血糖控制的作用。方法将我院与社区医疗中心共享电子档案的糖尿病患者120例作为实验组,无双向转诊管理的门诊糖尿病患者120例为对照组。实验组由我院内分泌科制定药物治疗方案。由社区医疗中心制定糖尿病的饮食、运动方案以及讲解低血糖的预防处理。对照组主要采取药物治疗以及一般性的饮食、运动方案知识普及。结果实验组定期体检、药物调整、饮食控制、运动管理、自我监测比率明显高于对照组。实验组的血糖、糖化血红蛋白以及并发症的发生率较对照组以及干预前后差别有统计学意义。结论双向转诊管理有利于糖尿病患者的血糖平稳管理,延缓或减少并发症的发生。
Objective To analyze the effect of bidirectional referral management on glycemic control in diabetic patients. Methods A total of 120 diabetic patients sharing e-files in our hospital and community medical centers were selected as the experimental group and 120 outpatients with non-bi-directional referral were selected as the control group. Experimental group by the Department of Endocrinology to develop drug treatment programs. Community health centers to develop diabetes diet, exercise programs and to explain the prevention of hypoglycemia. The control group mainly adopted the medical treatment as well as the general knowledge of diet and exercise programs. Results In the experimental group, the regular physical examination, medication adjustment, diet control, exercise management and self-monitoring rate were significantly higher than those in the control group. The experimental group of blood glucose, glycosylated hemoglobin and the incidence of complications than the control group and before and after the intervention was statistically significant. Conclusion Two-way referral management is conducive to the stable management of blood glucose in patients with diabetes, delay or reduce the incidence of complications.
